Coordinates services to embrace your physical, mental and social well being through the PACE program.  CSI-PACE is a unique health plan providing comprehensive medical care to Medicare/Medicaid eligible older adults.

Services are provided 24 hours a day, 7 days a week, 365 days a year!

• Physical and Occupational Therapy
